Program:   Located on Traverse City’s historic Grand Traverse Commons, Grand Traverse Pavilions is the region’s first and only public, nonprofit continuum of care. The Pavilions offers a wide range of intergenerational programs and services to an economically-diverse population. Fiscally self-sufficient for nearly two decades, The Pavilions is operated by the Grand Traverse County Family Independence Agency Board and owned by Grand Traverse County. Annually, The Pavilions and its affiliate Grand Traverse Pavilions Foundation provide more than $1 million in uncompensated, benevolent care to members of the community.

Eligibility:   15 years of age and older for Grand Traverse Medical Care. 18 years of age and older for all other programs and services, with the exception of Child Care Services, which enrolls infants, toddlers, pre-schoolers (academic year) and children 6 through 12 (

Cost:   Varies with type of service, duration of stay and applicable insurance coverage. Medicare and Medicaid accepted for applicable services. FIA voucher accepted for Child Care Services. Some financial assistance available to income-eligible individuals for Adult Day Services and Residential Services.

Services:   Alzheimer’s and Related Dementia Care: Complete continuum of care from early to latter stages. Homelike environment. Planned program of activities and intergenerational age-exchanges.Family-style meals. Access to Rehabilitative Therapy Services, Dry Land and Aquatic Health & Fitness Programs. Component of Long-Term, Skilled Nursing Care.Long-Term, Skilled Nursing Care - Innovative health care and residential environment. Registered Eden Alternative setting. Welcomes pets, plants, and children. Family-style meals. Planned program of activities and age-exchanges. Access to Rehabilitative Therapy Services, Dry Land and Aquatic Health & Fitness Programs.Adult Day Services - Social and recreational activities designed to stimulate participant to engage in meaningful interaction with peers. Assistance provided with personal care. Planned program ofactivities and age-exchanges. Lunch and snacks. Transportation available to and from home.Overnight Respite - Twenty-four hour staffing. Registered nurse supervision. Planned social and recreational activities. Family-style meals. Assistance provided with personal care. Access to Rehabilitative Therapy Services, Dry Land and Aquatic Health & Fitness Programs.Independent & Assisted Living - Studio and one-bedroom apartments as well as private and semi-private suites. Family-style meals. Planned social and recreational activities. Intergenerational age-exchanges. Access to Rehabilitative Therapy Services, Dry Land and Aquatic Health & Fitness Programs, Alzheimer’s and Related Dementia Care, Long-Term Skilled Nursing Care.Therapy Services - Rehabilitation for constituents. Physical, Speech, Occupational, and Aquatic Therapy. State-of–the-art Aquatic/Wellness Center on campus. Authorized provider of services to qualified members of community with physician referral. Health & Fitness Programs - Dry land and aquatic exercise sessions for constituents. Aquatic exercise classes open to community. Physician consultation recommended.
